HIV and complex needs referral information

Eligibility Criteria

To refer to our HIV services, in South East London the following eligibility criteria apply to our services in Lambeth, Southwark and Lewisham. 

The applicant will need to be:

• HIV+
• Require support to maintain to live independently
• Be living in or the responsibility of Lambeth, Southwark or Lewisham
• 16yrs +
  
  

Referral Process

 After we have received a referral we will need to book an appointment to visit the applicant to:

• Verify the details on the referral form
• Provide further information on our service
• Answer any questions,
• Assess risks
• Find out what support the applicant would like from us and, to see if we can meet their needs
• We will inform the applicant of the outcome of their referral in writing.  If we cannot provide support to the applicant we will try and suggest other services that may be able to help
