Here is a mini case study explaining how our Support Broker, Fran has helped a personal budget holder to achieve a more varied and independent life.
Background information:
- Miss Z (personal budget holder) was referred to Hestia’s SDS service by their social worker
- Miss Z is a 20 year old female of Pakistani origin with learning and physical disabilities
- Miss Z was informed by her local council that she has a personal budget of £520 per week to spend on services and support to meet her needs
What we did:
- Our support broker met with Miss Z and her family at their home
- One of Hestia’s staff volunteered as a translator to help Miss Z’s family to be fully involved in the process
- Our support broker explained to Miss Z and her family the self directed support process and discussed the outcomes Miss Z would like to achieve in her life
- Our support broker created a support plan with Miss Z that explains how she will spend her personal budget to meet her needs and outcomes
After the support plan was authorised by Miss Z’s local council, our support broker brought Miss Z’s support plan to life!
